Please attach of following documents (Copy):

PART-1

1. Company Trade License
2. Factory Trade License
3. e-TIN Certificate
4. e-VAT Registration Certificate
5. 2 Nos. Passport size photograph (MD/Chairman/Owner of the Company)
6. Completion Certificate at least 02 (Two) projects (not less than 50,000 sft.)
7. Factory Layout (Approved by Union Porishad or Pouroshova or City Corporation)
8. (a) Certificate of Incorporation (If Limited Co.)
   (b) Partnership Agreement (If Partnership Co.)
9. Certificate of Environment (from Department of Environment)
10. Fire License (from Fire Service & Civil Defense)
11. The nature of business must be specified in the trade license.
12. Machineries List with photograph
13. Photograph of Factory Building (Outside -2 & Inside – 2)

PART-2

1. Application form Tk. 50,000/- (Non Refundable)
2. New Membership Fee Tk. 2,00,000/- If your company Selected as a Member of SBMA (1 time)
3. Yearly Subscription Fee Tk. 1,50,000/- (Per Year)

If fulfill the above (Sl. No. 1 to 13) then you collect Membership Application Form & submit it for the same.
